About
The Take Me Home Program was introduced in Boyd County in 2020 by Boyd County 911, a program developed by the Pensacola Florida police department in 2003, to provide support to Law Enforcement agencies to assist those who may not be able to communicate or have forgotten their way home.
How it works
By filling out this form, and submitting it here, the information is placed into a database that can be accessed by Dispatchers and Responders. If law enforcement locates a person who cannot communicate where he or she lives, they can search this database, and reunite the person with their loved ones.
The system may also be used in assisting the locating of a missing individual, providing responders quick access to information such as
a photo, description, special needs, and who to contact once the individual has been located.
Submitting Information
Simply click here to download the form, and email it here. Once your information has been received, it will be
entered into the Take Me Home Database. If you have questions, or need assistance in submitting your form
please call the Boyd County 911 Center at 606-329-2191.
